5 Why's for Patients with Catheters
The 5 Whys technique is a simple but powerful tool that will reveal issues and will help you find the root causes of a problem. You should ask "why?" 5 times, or as many times as needed, after every response to get to the root of the issue. Remember, the purpose of the 5 Whys exercise is to find root causes, address the problem, and develop interventions to prevent further occurrence. Don't forget to print a completed copy for your records and to review during your QAPI/QA meeting.
