Introduction

Welcome to the Two-part Log that will help you reflect on the extent to which the McGill Annual Refresher Course for Family Physicians might impact your practice. The log consists of 23 questions (plus a question on consent).

Please complete Part I (questions 1-11) immediately after the course and Part II (questions 12-24) three months after the completion of Part I. Use the same link to access both parts of the log. You will receive email reminders to complete the log. Please disregard the reminders if you have already completed both parts of the log.

In order to facilitate learning and application of anticipated changes in your practice, we encourage you to keep notes about barriers and enablers (i.e., facilitating factors) to the implementation of the anticipated changes into your clinical practice. These notes will help you complete the second part of the Log.

Mainpro+ Certified Group Activity:

This one-credit-per-hour Group-Learning program meets the certification criteria of the College of Family Physicians of Canada and has been certified by the Office of Continuing Professional Development, Faculty of Medicine and Health Sciences, McGill University for up to 1 Mainpro+ credits.

You will need to complete BOTH parts of the Log to claim credits.

Please be reassured that your response will be held in the strictest confidence and that participation is entirely voluntary. Analysis and publication of results will be at the aggregate level only.
