Introduction

BACKGROUND INFORMATION
As part of the CAET Leadership Program, you are being invited to participate in a Stakeholder Survey to determine whether or not a name change for the Canadian Association for Enterostomal Therapy ( CAET)  and the Enterostomal Therapist/Enterostomal Therapy Nurse (E.T. Nurse) title is warranted. In 2014, as a part of the strategic planning process, the CAET Board of Directors identified a strategic priority to assess the name of the association and to look at whether the current name and name of the Enterostomal Therapist would be sustainable into the future. There have been concerns brought forward by CAET members and other stakeholders that the current names do not reflect the specialized role in wound and skin care, ostomy and continence care. In a recent CAET Member Survey, when asked if consideration should be made towards a name change, : 60% were in favour, 30% were neutral, and 10% disagreed.

THIS SURVEY
The purpose of this survey is to gather data to contribute to a rationale for a decision to be made whether to stay with the existing name for the Canadian Association for Enterostomal Therapy(CAET)/Enterostomal Therapy Nurses ( ETN) or to change the names. This decision will allow the CAET/ET Nurses to move forward with strategically desirable names and to create a clearer understanding, increased support, higher value and broader reach for nurses specialized in the three domains of wound, ostomy and continence care. This survey is open to all patients living with a wound, an ostomy and or a continence issue  from every province and territory across Canada.

RISKS
There are no foreseen risks associated with this survey. All responses will remain confidential and all data will be stored in a secure and password protected server. Data will be destroyed 5 years after the termination of the survey as per the CAET privacy policy.

BENEFITS
The  results from this survey may improve the understanding of whether or not a name change is warranted. The Canadian Association for Enterostomal Therapy (CAET) plans to use the results of this survey to support the name change decision and will work to improve the value proposition of the names no matter what the outcome.

EXCLUSIONS
You will not be considered for this survey if you are not a person living with a wound, an ostomy or continence issue.

CONFIDENTIALITY
All information obtained during the course of this survey is strictly confidential and your anonymity will be protected at all times. There will be no personal identifiers on any data. Confidentiality - Data will be stored in locked files and will be available only to the principal investigator and research team.You will not be identified in any publication or reports.

VOLUNTARY NATURE OF SURVEY/FREEDOM TO WITHDRAW OR PARTICIPATE
Your participation in this survey is voluntary. You may withdraw from this survey at any time and your withdrawal will not affect your involvement with the CAET.

Consent will be implied by participant’s completion and submission of the completed survey via Survey Monkey’s online site. All personal identifiers will be removed from the data prior to download by the researcher from the Survey Monkey database.
