THEREFORE, IT IS IMPORTANT THAT YOU DO NOT GIVE YOUR HIV STATUS OR OTHER PERSONAL HEALTH INFORMATION ON THIS FORM IF YOU DO NOT WISH FOR THE INFORMATION TO BE PUBLIC.
