Summer 2 Extra Curricular Clubs

Please answer the following questions if you would like a place for your child/children at any of our lunchtime or after school clubs. Clubs are free and will begin 14th June 2021.

2.Which class is your child/children in?(Required.)
3.Would your child/children like to attend our Healthy Lifestyle Keep Active lunchtime club? Please select which days below (these will be done in class bubbles outside). If you have more than one child please indicate which child wants to attend using the other section
4.Which after school clubs would your child/children like to attend? Please ensure that you only put your child/children into the clubs available to their class.(Required.)
5.Will your child/children be collected or walk home?(Required.)
6.Please give your name as authorisation for your child to attend clubs (confirmation of your child's club places will be sent by the 11th June 2021)(Required.)
