Banff & Macduff Youth-Led Club Project: Participation & Parent/Carer Consent Form

You or your child have been invited to participate in the Banff and Macduff Youth-Led Club Project.

This form is for:
  • gaining consent preferences from a parent/carer for young people of 16 or under to participate.
  • gaining contact preferences for over 16s who will be participating.

1.I have read the Project Info Sheet or the nature and purpose of the project has been fully explained to me.

I also understand that participation in this project is voluntary and that there will be no adverse consequences for any participants if they do not participate.

Lastly, I understand that any participants are free to withdraw from the Panel at any time before the findings are reported.(Required.)
