This survey is confidential. Your name will ONLY be used by the external evaluators to link your past, current, and later survey responses. Your personal responses will NOT be shared with anyone at your school or on the Out Teach staff.

Please be as accurate and honest as you can in your responses. This allows the Out Teach team to use the results of this survey for multiple purposes, including tailoring professional development offerings, selecting partner schools, and tracking program outcomes over time. The Out Teach team makes decisions about school selection and professional development offerings based on how our organization’s resources align with your school’s genuine needs, not on the basis of how “good” or “correct” your ratings are.
