About This Survey

Working in partnership with Canton City Public Health (CCPH), the Ohio Department of Health (ODH) Bureau of Environmental Health and Radiation Protection  created this survey to  gather information from the community/neighborhoods near the Republic Steel site located on 8th Street NE in Canton. This information will help CCPH and ODH know more about the community’s environmental and public health concerns. This will also help ODH conduct a public health assessment and prepare a Public Health Consultation  for the community in cooperation with Canton City Public Health and the Ohio Environmental Protection Agency (Ohio EPA). There are 18 questions in this survey. Some questions require an answer while others are optional. Questions 1 - 4 ask you about your concerns and knowledge about the Republic Steel site.  Questions 5 – 10 ask you about some of your personal behaviors that might have affected your exposure to contaminants from Republic Steel. Questions 11 - 18 ask you about your demographics and contact information.
This survey is completely voluntary and designed for people who are 18 years and older. It is designed to be taken by a single person. If there are multiple people in your household who wish to respond, they should each take this survey separately. It is written in English and should take 5 minutes or less to complete. If at any time you decide you do not want to provide answers to this survey, you can leave the survey.
ODH may share the combined (no individually identifiable) results of this survey with  CCPH and Ohio EPA for the purposes of better understanding community concerns regarding Republic Steel. Individual information like name, address, and phone number will only be available to ODH if you choose to provide them. ODH will not share individual information with outside agencies or publish individual information in any reports.
