The Pandemic has heightened the importance of a good internet connection at your home. Grant funding has been made available to ensure that all Colorado students can access online resources. Filling out this short survey will allow us to improve internet connectivity where it is most needed.
1.Your Name and Address: We understand that this information is sensitive. Your information will never be used for marketing purposes or shared with outside organizations that are not directly providing the internet to your home.(Required.)
2.Are you an employee of a School District or BOCES?
3.School District in which you reside
4.Does your internet perform well when your family is at home using it? (It does not have frequent problems with sites loading, videos playing, lost connections etc. ?)
5.How many people in your household use the internet?
6.Does your internet provider have other packages with different speeds/bandwidth amounts for your internet connection?
7.If there is a better package, have you upgraded and if not, why?
8.Do you know who your Internet Service Provider (ISP) is?
9.Do you have multiple Internet providers you can pick from?
10.Do you know what your internet speed is?  Ex: (10Mbps, 20Mbps, 100Mbps, etc)
