Partnership in Care: Supporting Patients and Caregivers After Early-Stage Dementia Diagnosis Webinar

This NCECE webinar will explore how patient support needs evolve after an early-stage dementia diagnosis and care planning strategies that effectively engage informal caregivers as key partners.
Participants will consider practical approaches to enhancing both patient care and caregiver well-being during the critical period following a dementia diagnosis.

At the conclusion of this webinar, participants will be able to:
  • Understand common challenges experienced in early-stage Alzheimer's disease and related dementias and their impact on patient care requirements
  • Explain the value of care coordination approaches that meaningfully engage family caregivers and support persons as essential collaborators within an interprofessional team
  • Utilize shared strategies to evaluate and enhance caregiver wellness, particularly during the transition period following a dementia diagnosis

This session will feature insights from Dementia Care Aware, a training and support program focused on enhancing dementia care in primary care teams.

This webinar will be held on Wednesday, December 3rd, 2025, 2-3PM ET.
