Nomination Instructions

Use this form to nominate your candidate for the Future of Life Award. Please explain how your candidate(s) has helped (indirectly or directly) save thousands or millions of lives, or helped make today dramatically better than it may otherwise have been. Please include any relevant websites (URL's) that would help us evaluate your nomination. You can upload files for additional support.

You can nominate up to three people for one award. If you have other ideas for candidates, please resubmit the form as needed. 

If we decide to give the award to your nominee, you will receive a $3,000 prize from FLI for your contribution!
