11.Please describe your health center’s goals for participating in the Colorectal Cancer Screening Learning Collaborative. Why is this important to your organization?
12.Currently, what are you using to screen for Colorectal Cancer (i.e. FIT, Cologuard, colonoscopy)? Do you have plans to change the screening method in the next 2 years?
13.Are there factors that have affected your health center’s screening rate and process? (please elaborate)
14.Where do you refer patients for colonoscopies? If you are aware, what is the status of your colonoscopy provider (backlogged, only providing diagnostic colonoscopies, active etc.)?
15.For multi-site health centers only - The CDC is requiring participating health centers to report data and activities by site.  How will you ensure that tests of change can be carried out at multiple sites?  How many sites would you choose to include in the initiative?

17.Describe what data reports and tools your health center has access to for monitoring colorectal cancer screening and other improvement projects, e.g. DRVS, population health reporting tools, EHR generated reports.  Please name your current EHR. Are there any plans to change EHR in the next 2 years?
18.Describe your ability to provide the time required for a champion-led multi-disciplinary team to participate in improvement activities related to colorectal cancer screening, and the additional time required for internal activities like staff debriefs and data analysis.
19.What other quality improvement projects is your health center working on/prioritizing at this time?
20.In Massachusetts, there are disparities in screening rates for colorectal cancer across ethnicities and regions. One of the objectives for this learning collaborative is to reduce and ultimately eliminate disparities in CRC screening. Has your health center identified any disparities in their colorectal cancer screening data and if so, what steps has your health center taken to address these inequities? Are there any other projects with an equity focus being carried out at your health center?
