EAP Client Satisfaction Survey

This survey is being conducted by UCA BeWell and is intended to collect feedback regarding your recent experience with the Arkansas Employee Assistance Program (EAP) provided by UAMS. The survey will take approximately 3 to 5 minutes to complete and your responses will remain anonymous unless you choose to identify yourself. We are committed to maintaining your privacy when utilizing EAP.
