Consent to Participate in Research

 

PROJECT TO IMPROVE IN-HOSPITAL SUPPORT FOR PERSONS WITH DISABILITIES

 

INTRODUCTION AND PURPOSE

Our names are Rosina Isabella and Paul Benson.  We are undergraduate students, ourselves with disabilities, in the School of Disability Studies at Ryerson University, working with our faculty supervisor, Dr. Kiaras Gharabaghi, School of Child & Youth Care and the John C. Eaton Chair of Social Innovation.  We would like to invite you to take part in our research study, which concerns hospital and other healthcare related services delivered to persons with physical disabilities. Our aim is to capture information on your experiences regarding the adequacy of those services and to identify areas in which they can be improved.

 

WHAT YOU ARE BEING ASKED TO DO

You are being asked to voluntarily complete this on-line survey. It involves questions about your experiences receiving services as a patient in hospital settings and should take about 15 to 30 minutes to complete. Some questions are open-ended and you will be able to provide your thoughts in a text box. Other questions are yes/no or multiple choice questions. In order for all of your answers to be collected you must go to the end of the survey and click ‘submit survey’. This will demonstrate your full consent to participation.

 

POTENTIAL BENEFITS

There is no direct benefit to you for taking part in this study. It is hoped that the research will provide better information to health care providers and policymakers about the experiences of persons with disabilities when visiting hospitals.

 

WHAT ARE THE POTENTIAL RISKS TO YOU

Some of the survey questions may make you uncomfortable or upset or you may simply wish not to answer some questions.  You are free to decline to answer any questions you do not wish to answer, or stop participating at any time by closing your browser. If you close your browser before getting to the end of the survey and do not confirm your consent to participate at the end of the survey by clicking the ‘submit’ button your information collected up to that point will not be used.

 

A list of online, free support groups are listed here in case you feel you need support during or after the survey is completed. You could also contact the researcher/s if you would like us to find supports for you. Please note if you do this your identity will be disclosed to the researcher. Please print this page or write down the contact information in case you want to access this information once you complete the survey.

 

YOUR IDENTITY WILL BE ANONYMOUS

The survey is anonymous and as such will not be collecting information that will easily identify you, like your name or other unique identifiers. Although your Internet Protocol (IP) address can be tracked through the survey platform, the researcher/s will not be collecting this information. Your IP address may be observed only to ensure that one individual is not completing the survey multiple times.
