Please help us get to know your child and let us know when your child would like to attend our program.

How comfortable are you with your child being outside in all types of weather? Taking risks like climbing trees, playing in the creek, be
around open flames?
3.
Please briefly describe anything we should know about your child's interaction with other children as well as with adults.
4.
Please briefly describe anything we should know about your child's level of independence (dressing, toileting, etc).

Have you left your child with a babysitter or dropped your child off at group care settings such as preschool, daycare, etc? Please describe. How did the good-byes go? Were there any challenges for you or your child in this program?

What are your child’s favorite things to do? What does your child find challenging? When your child is having a hard time, what helps to calm him/her down?

Does your child have any special needs or learning differences, or diagnosed or undiagnosed emotional or cognitive challenges? If so, please describe/explain. Please note any evaluations or therapies your child has received or is currently receiving. Also, any allergies to foods or insect bites and dietary restrictions?
