This survey is to find out more about what you think about the activities and staff at the REACH Ashland Youth Center. Your responses will help us to understand what is working and what could be improved.

The survey should take approximately 10 to 15 minutes to complete and is confidential (no survey will be connected to any one person).

Once you finish the survey, you have the option to enter a raffle for a $100 gift card. Your contact information will not be linked to your survey responses. For the raffle, 15 winners will be selected from each survey respondent who provides a complete email or cell phone number (up to 200 survey participants)! Each winner will get a $100 gift card.

Raimi + Associates (R+A) conducts the annual evaluation for REACH. Only R+A staff will see your contact information and they will only use it to send your gift card. R+A staff will also analyze all survey data and will ensure that no identifying information is included when the results are presented to REACH staff.
