Exit Membership Interest Form Question Title * 1. Thanks for your interest! What is your name and who encouraged you to apply? Question Title * 2. How do you plan to use the space? Competition Preparation Personal Pole Practice Floor space for rolling around, yoga, stall bars, fly gym etc. Teaching classes Other (please specify) Question Title * 3. What is the best way to communicate with you about what's happening at the studio (new classes/workshops/reminders/etc.)? Facebook Instagram E-mail Website Question Title * 4. What are your thoughts on safety? I feel like I can practice safely without hurting myself or others. I always ask for a spot when I feel unsure of myself. I feel like pain is a part of the process and if I get a few bumps and bruises, I'll be okay. I don't know most of the names of the moves I've learned, and I don't know if I am doing them right quite yet. I love falling on my head! Question Title * 5. We love it when people use the studio to practice! Chances are there may be someone else practicing at the same time as you, how does this make you feel? I like to have a serious practice and keep to myself, I may say hi but I'm either pretty shy or too focused to talk. I am a bit of an extrovert, so I'll want to introduce myself and try to get to know whoever is in the studio a bit more. I think I forgot something at home, I'll be right back... Question Title * 6. As a member of the studio, you have access to the building, but this does not apply during class times. So you cannot use the studio for personal practice if a scheduled class is in session. I understand, I promise not to bang on the door or do burnies in the parking lot until someone lets me in. I understand, but do not like it, can we change this? Thanks, but I've changed my mind. Question Title * 7. Part of being a member means that you can bring your friends, with a signed waiver and fee, but we cannot allow children or pets (unless they are service animals). Yes, I will make sure that my friend signs the waiver and pays the $10 day fee. No pets or kids. Got it. But... can you make exceptions? I couldn't find a sitter, and my hedgehog gets lonely in the car. Question Title * 8. We want everyone to come into a nice, inviting, clean space. This means that after you are done with your personal practice we expect members to wipe their poles and mats. This also means no outside food or drinks in the studio, or outside shoes on the studio floors please. I understand that as a member I will "leave no trace." You won't even know anyone was here! If I am too tired I will not do those things, sorry. Question Title * 9. (For Teachers only) All people who are interested in teaching at our space must be members, and must supply proof of insurance. I accept the fee, and will supply my insurance. I have questions. Can we talk? I don't plan on teaching. Question Title * 10. You made it to the end! Now if you could please give us your contact info where we can reach you. (Please include a phone number!) What time works best for us to give you a membership orientation? Also, please let us know if you have any other questions or concerns. Done
