Title V Maternal and Child Health (MCH) Services Block Grant
We Need Your Input!
Each year, the KDHE Bureau of Family Health submits an application for the Title V Maternal and Child Health (MCH) Services Block Grant, which provides support for gap-filling MCH services across the state. Before submitting the application, the Bureau of Family Health is required to make a draft of the application available to the public to gather input.
We’ve created this survey to help gather Kansans’ opinions and perspectives on the draft of the 2023 Title V MCH Block Grant Application and 2021 Annual Report. We will use the results of this survey to make any improvements to the document before submitting it.

As you go through this survey, the questions will guide you through the key information found in the application and annual report. Links will be provided to sections of the application and annual report, so that you can easily navigate through them.
It is our hope that you answer as many questions as you can, as your answers will help us to understand how well the application describes ongoing efforts to improve MCH in Kansas. The survey will allow you to skip any questions you do not wish to answer.
