The Indigenous Transitions Navigator or bami'iwe (Ojibwe for s/he takes care of people, helps people, gives care) position will support CKHA in its commitment to reconciliation. The role has been developed from guidance from the community and local Indigenous partnerships.

ESSENTIAL QUALIFICATIONS (Knowledge, Skill, Experience)
-Minimum of five years' work experience in an Indigenous health and or social service organization, hospital or community-based case management role
-Knowledge of biopsychosocial assessment models, and social determinates of health
-Nursing or social work/social service background (preferred)
-Experience working with First Nations community and urban indigenous community
-Preference will be given to an Indigenous person from a local indigenous community
-A member in good standing with current registration with a regulated professional college (preferred)
