This survey is intended to study the opinions and knowledge of librarians about authentication and access of licensed library resources in hospitals, health systems and institutions providing patient care. All data will be reported in aggregate and individuals and institutions will NOT be identified. Definitions of terms used in this survey:

I.T. - The Information Technology Department; the group at your institution (in-house and/or outsourced) that is charged with establishing, installing, monitoring and maintaining information/computer technology systems (hardware, applications, networks) services and their support. 

EHR - Electronic Health Record - A real-time patient health record with access to evidence-based decision support tools that can be used to aid clinicians in decision making. (U.S. ONC) A repository of electronically maintained information about an individual’s health status and health care and may be and inpatient and/or an ambulatory.
