Thank you for your contributions to shaping Kansas's 'Find my Vaccine' tool!

Kansas Department of Health & Environment (KDHE) will launch a ‘Find my Vaccine’ tool on www.kansasvaccine.gov, which will enable Kansans to search for providers in proximity to them, see which providers have been allocated doses in the past week, and access provider contact information. This tool is critical to lowering the barrier to entry to Kansans across our state by connecting providers with those seeking to be vaccinated.

We seek your willingness to be included in the first version of 'Find my Vaccine' and ask for your support to provide information necessary for inclusion via this input form. Your input on this form will directly inform what information is available about your location(s) on 'Find my Vaccine'. This form should take ~10 minutes or less to complete. All providers who opt-in to the tool will have the opportunity to update their information as the tool evolves.

KDHE will be introducing a new public-facing tool that will support expanding vaccination efforts. Kansas is preparing to move into Phase 2, adding Kansans 65+, individuals in congregate settings, and those working in high-contact critical settings to the broad swath of Kansans who need to know where they can go to get vaccinated.


In response to this transition, KDHE will launch a ‘Find my vaccine’ tool on www.kansasvaccine.gov, which will enable Kansans to search for providers near them, see which providers have been allocated doses in the past week, and access provider contact information.


KDHE's request
KDHE hopes that all enrolled providers who intend to vaccinate the general public will elect to opt-in to this tool.  The map itself will clearly indicate which providers have been allocated vaccine within the past week to minimize an overload of outreach from the public if a provider does have inventory on hand.

This opt-in survey is meant to create the base layer of information that you would like to be visible to the public for the location(s) you are publishing. You will be able to update this information over time.


What to expect in the first version
The tool will evolve over time. KDHE may expand the information that is displayed.  KDHE may decide to pause the tool, if other systems such as the forthcoming CDC VaccineFinder public-facing system goes live and better serves the need (which will be based on the daily inventory reporting providers submit). All will have an opportunity to provide ongoing feedback on how it can be improved.

In its first iteration, the map-based ‘Find my Vaccine’ tool will show the broad footprint of all providers who have enrolled to administer the vaccine.  For those providers that do opt-in, the following information will be required & available for display when Kansans click on any one provider location dot on the map:
  • Provider location name 
  • Provider location type (e.g., Local health department, pharmacy)
  • Provider location address
  • Provider location contact information (phone, website, or email)
  • Other optional info provided in this form may also be displayed
