Welcome to the KAMHA 2021-2022 Coaching Feedback Survey

The following survey has been developed with the purpose of evaluating our Representative Coaches within the Kingston Area Minor Hockey Association. Your input will help to:

1) Provide Feedback which will encourage our coaches to improve and contribute to their development and

2) Assist in the coaching selection process

Although your name and email are required for verification - your responses will be kept anonymous and confidential. 

In the effort to get player feedback directly there is a short, 5 question YES/NO survey for your child to answer at the end of this survey.
