Welcome! Thank you for considering telling us more about your experiences with transformation. This survey is part of ongoing studies by researchers at the Institute of Noetic Sciences looking into the nature of transformation, and experiences, practices, and other factors involved in the transformative process. Your responses to this survey will be used to help researchers learn more about how people transform, and the practices and activities that foster transformation.
We hope that responding to this questionnaire will be an interesting experience for you and will provide an opportunity to reflect in depth on your experiences and insights. We estimate it will take between 30 and 90 minutes to complete, depending on how detailed your responses are and how much time it takes you to answer the multiple-choice questions. The survey will ask you to describe in detail a transformative experience, and will ask you many questions about your thoughts, feelings, beliefs, and background.
Please find a time that you can complete the survey all at once. It must be done in one sitting (in other words, you will not be able to come back and finish it later if you have to stop once you get started).
We realize that surveys are limited in their capacity to reflect the breadth and depth of experience, and that some questions may be difficult to answer. Whenever possible, please choose the answer that most closely reflects your experience. Please respond with how things actually are for you, rather than what you hope for or strive for. Also, we recognize that some questions are very similar.
This survey is completely voluntary and anonymous. It is completely up to you whether or not you fill it out. If you do fill it out, you have the right to skip any question you do not want to answer. However, it will be most helpful to us if you fill out every question as openly and honestly as you can. Please be aware that completing this survey may cause you discomfort, either due to the time it takes to complete, or the questions themselves. If you do choose to provide contact information, you will risk a loss of privacy (because you are telling researchers about yourself), but within the limits of the law, we will keep the information you provide strictly confidential.
We will not collect any information that can be used to identify you unless you choose to give us that information. If you have any questions or comments about the survey, there will be a space at the end for you to provide them.
