In order to gauge the number of applications we will receive for the Request for Proposals (RFP), it is required that all new applicants complete and submit this Notification of Intent to Apply by the deadline of Tuesday, December 5, 2017. 

* 1. Please provide the contact information for the primary contact person for the purposes of this RFP.  Please be sure that the primary contact is authorized to correspond on behalf of the organization.

* 2. Please provide your Organization's DUNS number.  All applicants must have a DUNS number and be registered with the System for Awards Management (SAM) prior to submission.  If you are in the process of applying for your DUNS, please provide that information below.

* 3. Which CNCS Focus Area will your program primarily address?

* 4. Will your program address any Special Initiative?

* 5. Program locations where AmeriCorps members will serve:

* 6. How many slots do you plan on requesting?

* 7. Provide a brief list of the activities AmeriCorps members will be participating in.

* 8. Provide a brief description of the target population/beneficiaries of your AmeriCorps program activities (e.g. third graders at a certain reading proficiency).
