Proxy Delegate Nomination

For the 2024 Churches of Christ in Queensland AGM on 5th of November, delegates are required to attend from each of our affiliated churches. By completing this survey, you are registering another individual (proxy) to take the place of one of your delegate representatives to attend this year's AGM on behalf of your church.

Please note that an Affiliated Church must complete the Delegate Registration Form noting the names of their delegates before a delegate can indicate via this form a proxy on their behalf.
