1. INSTRUCTIONS
Please make sure you complete each survey in FULL. It is very important to complete Question 1 to identify yourself, in order to track completed evaluations.
***NOTE***
Each instructor necessitates a separate evaluation; these evaluations are not broken down simply by course. I.E. The Aggression seminars run from Dec 14 - Feb 1, and each of the 6 seminars have a difference instructors, therefore 6 evaluations are required (one per instructor) even though all are teaching "Aggression".
