Introduction and Instructions
Thank you for your interest in becoming a QuakeCoRE Industry Affiliate.
Please read the information below before submitting your request, this form should take between five and ten minutes to complete. Once your request has been submitted you will receive an email from QuakeCoRE confirming the status of your application shortly.
Who
Any stakeholder or industry practitioner that is engaged with QuakeCoRE and would like to acknowledge this relationship by being named as an Industry Affiliate.

What
As a QuakeCoRE Industry Affiliate you will be able to:
o Apply for a travel grant to attend the QuakeCoRE Annual Meeting
o Receive priority registration for the QuakeCoRE Annual Meeting
There are no reporting requirements for QuakeCoRE Industry Affiliates
How
Applications are due by Noon on 20th October 2017
