Participant Information Statement

TITLE: Assessing Q fever understanding in general practice

Principal Investigator: Dr Penny Hutchinson (Darling Downs Hospital and Health Service)

We are seeking your participation to complete a survey on Q fever. Below are answers to some questions you may have about this study.
Study Objective
Through this study we are aiming to ascertain knowledge, attitude and behaviour around Q fever amongst general practitioners.

Outcomes from the project will provide important information around current knowledge on Q fever from a variety of different general practices across the West Moreton and Darling Downs regions to compare with the current epidemiology of this disease and to identify where information may be needed in future.

What information are we looking for?
In addition to collecting information about your age, sex, years in practice, and years in current practice, we will be asking a series of questions about characteristics of current and past practice locations; understanding around Q fever; and interaction with patients about Q fever.

What will we do with the information we get from you?
We are collecting information from GPs practicing in the catchment of Darling Downs and West Moreton Primary Health Network.  We will put together the information that we collect from all participating GPs to identify levels of knowledge as well as attitudes and behaviours around Q fever. We will not be looking at information from each participant separately.

How long will it take?
The survey should take no more than 20 minutes.

Who will see the information we collect you?
Only the principal investigator and researchers from the University of Queensland doing this study will see the information collected from the survey. Any information that is obtained for this study that could possibly identify you will remain confidential and will be disclosed only with your permission or as required by law.

We will not include your name when we collect information for this study.  This means that no one can be identified from the reports that we write presenting the results.  We may present the findings of this study at conferences or in medical journals but no person will be identifiable from these reports.

Privacy disclaimer:
This survey is being conducted using SurveyMonkey which is based in the United States of America. Information you provide on this survey will be transferred to SurveyMonkey¹s servers in the United States of America and Luxembourg. SurveyMonkey’s Security Statement can be found at https://www.surveymonkey.net/mp/policy/security/. By completing this survey, you agree to this transfer. If you do not agree to the transfer of your information to SurveyMonkey, please contact the Research coordinator for an alternate document process.

What if I change my mind?
You are free to withdraw from the study at any time without penalty. If you close the browser window or otherwise withdraw before reaching the end of the survey, all earlier survey answers that you have provided will be deleted and will not be included in the study analyses. Once you have completed and submitted your responses they will be anonymous and unable to be identified for removal.

Benefit
While there may be no immediate benefit to you in being involved in this study, your input will help us to provide important information on the current knowledge of Q fever. This will result in the development of comparisons to current epidemiology to identify knowledge gaps and may lead to a subsequent learning module on Q fever, available to you.
Who is conducting this study?
This study is being conducted by the Public Health Unit of the Darling Downs Hospital and Health Service and a team of researchers at the Rural Clinical School, The University of Queensland. The research is being led by Dr Penny Hutchinson, a public health physician based in Toowoomba.

What do you have to do to take part in the study?
If you are happy to take part in the study, please complete and return the consent form and the survey that are attached.  Also enclosed is a reply-paid envelope to return the completed consent form and survey to us.

What if I have any questions?

This study adheres to the Guidelines of the ethical review process of The Darling Downs Hospital and Health Service and the National Statement on Ethical Conduct in Human Research. You can contact the research team with any questions. Anything else?
This is an important topic, one which may have a great influence on how Q fever is recognised and managed in our population. Your contribution will be fundamental to our investigation.

Thank you for your assistance with this study.

Yours Sincerely,

 Dr Penny Hutchinson
