This evaluation form is PART of an ongoing overall effort to strengthen and improve the ministries of Bethel Church.

Your position and responsibility are very important in conveying truth, wisdom and insight in a spirit of grace and discernment. Please read the following scriptures followed by a season of prayer before completing this form. (Read I Timothy 3:1-7; James 1:19-25; and Hebrews 13:15-21)
