It can be difficult to meaningfully answer the question: "What's your culture like?" By assessing observations of your workplace, this survey is designed to help you to instead answer the question: "How well does your culture function?"
This survey is only as effective as the authenticity of your responses. Please provide responses based on your lived experiences, observations and perspective of the existing state of your workplace - not the ideal or desired version.
Any individual responses collected will be de-identified and only shared to the email you provide. Upon completion, you will receive a brief diagnostic summary to help you reflect on and articulate your perception of your workplace culture's strengths and opportunities.
