2026 Community Health Needs Assessment (CHNA) Stakeholder Survey

Purpose: Damar Health is a community health center. Every three years, we ask people in our area about their health needs. This is called a Community Health Needs Assessment. We want to hear from everyone—especially people without insurance or who have trouble getting care.

Your answers will help us:
  • Know what health problems matter most.
  • Understand what makes it hard to get care.
  • Plan better services and work with other groups to help.
