PreparED Information Sheet & Consent

Thank you for interest in the PreparED training. Please read through the following information prior to starting the training.
PURPOSE

Eating disorders are complex, psychiatric disorders. PreparED offers a broad overview aimed at familiarizing students, trainees, and generalist clinicians with basic but essential information on eating disorders. This 70-minute training will cover four modules of information: Assessment, Medical Complications, Treatment, and Military Relevant Considerations.

This research study aims to:
  1. Examine baseline knowledge and attitudes about eating disorders among Defense Health Agency (DHA) medical providers
  2. Evaluate the usefulness of PreparED in increasing providers’ knowledge of and confidence in treating eating disorders.
PARTICIPATION

You are being asked to participate in this research study because you are a DHA employee who provides care to military-connected individuals and their families. You are being asked to complete two (2) surveys approximately 10 minutes and the PreparED training, which is estimated to take 70 minutes. You will not be asked to provide personally identifiable information at any time during your participation.

Participating in this research study is entirely voluntary, and you are free to skip any questions or procedures or withdraw from the study at any time without penalty or loss of benefits to which you are otherwise entitled. To withdraw, please quit the survey at any time.

ALTERNATIVES TO PARTICIPATION

You have the choice not to participate in this research study. If you do not wish to participate in the research study, you may complete the non-research version of the PreparED training here. At the end of both the Research and Non-Research option, participants have the opportunity to complete a post-training survey that will be used for program evaluation. Program evaluation helps to determine whether PreparED is effective in increasing providers’ knowledge, confidence, and comfort with eating disorders. The data collected from the Research Pre-Survey and both the Research & Non-Research post-surveys will be shared with the training team and may inform future iterations of the PreparED intervention.

The questions on both the Research & Non-Research Post-Survey are are identical; however, the difference between the two options is that the data collected via the research survey may be presented at scientific or professional meetings or published in scientific journals.
RISKS, STRESS, OR DISCOMFORT

The possible risks to you in taking part in this research include feelings of discomfort related to the topic of eating disorders. Although this is unlikely, the research team will take appropriate measures to secure the data collected to mitigate this risk. Your confidentiality will be kept to the degree permitted by the technology being used. We cannot guarantee against interception of data sent via the internet by third parties. Additionally, data may exist on backups and server logs beyond the timeframe of this research project. We anticipate that your participation in this survey presents no greater risk than everyday use of the Internet.

No identifiable information will be collected to protect your identity as a research participant. Additionally, your name or other private information will not be used in the event that this research is published.
PRIVACY INFORMATION

Privacy/Confidentiality/Data Security

Deidentified data will be stored on the study's Google Drive. Data will be downloaded into a password-protected file on a secure, password-protected study computer for analysis. Identifying information such as your name, contact information, or IP address will not be collected. Data will be maintained for seven years after the completion of the study.

Please note that the surveys are being conducted via SurveyMonkey, which is not affiliated with the Uniformed Services University of the Health Sciences or the Department of Defense. It has its own privacy and security policies, which you can find on its websites.

Sharing De-identified Data Collected in this Research

De-identified data from this study may be shared with the research community at large to advance science and health. We will remove or recode any personal information that could identify you before files are shared with other researchers to ensure that, by current scientific standards and known methods, no one will be able to identify you from the information we share. Despite these measures, we cannot guarantee the anonymity of your personal data. Your deidentified data collected as part of this research may be stored and used for future research by the study team. No personal identifiers are included in this data so the data cannot be linked back to you. The data will not be given to anyone else for future research studies, even if all information that personally identifies you is removed.

STATEMENT OF CONSENT

If you have read this form and have decided to participate in this project, please understand your participation is voluntary, and you have the right to withdraw your consent or discontinue participation at any time without penalty or loss of benefits to which you are otherwise entitled. The alternative is not to participate or to complete the non-research version of this training here. You have the right to refuse to answer particular questions. Your responses to items on this survey will not be monitored in real-time, nor will a member of the research team contact you regarding your responses to any item on the survey. The results of this research study may be presented at scientific or professional meetings or published in scientific journals. Your individual privacy will be maintained in all published and written data resulting from the study.

This consent form will be kept by the researcher for seven years beyond the end of the study.
