Objectives and Summary:
I am currently conducting a national survey to better understand the experiences of people living with blindness, deafblindness or partial sight when accessing healthcare services and interacting with healthcare providers in clinics, hospitals, medical offices, and other healthcare settings.
Your responses will help identify barriers, highlight positive experiences, and uncover opportunities to improve accessibility, communication, dignity, and independence within the healthcare system. The findings will also inform improvements to programs and services that support the vision loss community.
Together with a team comprising people with a lived experience of vision loss, we have developed the survey available through the link below. We hope you will take a few minutes to complete it. The survey should take approximately 30 to 45 minutes of your time and your participation will greatly help other members of the community.
Participation is entirely voluntary and will not impact the membership services you currently receive from any organizations now or in the future. You may skip any question that makes you feel uncomfortable and answer only those that apply to you.
Data Storage and Confidentiality:
Your responses are completely anonymous, and the data collected will remain private. Your name is not attached to the survey. Only the research team will have access to this data, and it will not be given to anyone else. All study data will be stored in a password protected data base and the results will only be reported in aggregate. Survey response data will be securely destroyed after 5 years in accordance with institutional policies and guidelines.

Please include your name, a phone number where you can be reached, preferred time to call, and your preferred language ( English or French only). A member of our team will contact you within 2 business days, weekends excepted.
