Corentus Team Development Program for Internal Practitioners

If you would like to fill out chunks overtime PLEASE email Janice.Caillet@Corentus.com and ask for your own special questionnaire link (email link sent within 24 hours). Using this method, every page of your form will be saved (after you click on 'Next' on the bottom of each page). To continue where you left off, go back to the link from the email.  Using this method, you will also be able to return to previous pages of the form to make any changes you wish.

MOST IMPORTANTLY:
This questionnaire is for us to get to know one another better.  We are not worried about grammar or even sentence/paragraph structure.  Please write as if we were in conversation with one another -- fluid and from the heart.

On this page there is a link to the .pdf of this application so you will be able to view all of the questions in this application to be better prepared with your answers prior to posting them here.  We highly recommend typing your answers to some of the questions that need lengthier answers first and then copy and paste into this form.

In brief, there are four main sections to this application:
1) About You (the basics)
2) Work Experience (working with teams)
3) Education & Training
4) Your Goals

Please don't worry about grammar or convention, just write.  We want to get a better understanding of you and your experiences.

NOTE!!
The deadline for all applications is: December 1, 2019.
Practitioners are accepted into program on a rolling basis.  Once the cohort is filled (maximum of 18 people) it is filled and sold out. Therefore, please apply early.   Questionnaires received after the course is full or beyond the due date will be considered for future cohorts.
