Your contribution to the following survey from Johnson County Case Management (JCCM) is requested.

The purpose of this survey is to provide you with an opportunity to express your thoughts regarding services rendered by JCCM. 
Please complete the survey based on your typical interactions with Johnson County Case Management staff.

1.JCCM assists clients in gaining access to appropriate and necessary services in a timely manner (for example: returning phone calls, response to requests, completing paperwork, etc.)
2.Case Managers are willing to problem solve which includes exploring all options.
3.JCCM staff are knowledgeable and produce quality work.
4.JCCM provides high quality customer service (staff are personable and professional).
5.The frequency of contact with JCCM staff members is as follows:
Members-Case Manager sees you face to face at least every three months.
Providers-Case Manager solicits information regularly including phone contacts and regular meetings.
