1.What library do you work for?
2.Do you know if your library's headquarters or main branch is geographically close to any Correctional Institutions? If yes, which institution would be closest?
3.Does your library currently provide any kind of reading support or services to a Correctional institution? This could include providing books, printouts, research assistance or literacy support.
If yes, please state which institution you work with, and what the support or services look like.
If not currently providing services, see the next question.
4.Did your library historically provide any kind of library supports or services to a Correctional institution? If yes, please state what services were provided and for how long; and, when they were discontinued, and why.
5.If your library currently has a connection to a Correctional institution, do you wish to collaborate with the Prison Library Subcommittee to continue providing books or related support?
6.If your library is not currently involved in providing books or supports to a Correctional Institution, would you like to be connected to SLA's Prison Library Subcommittee?
