By signing this declaration, I do hereby declare that I give my consent to the Bulgarian Tennis Federation, Reg. № in Bulgaria 000708469, having its registered office at 3-5 Nezabravka str., Sofia, Bulgaria, to administer my personal data, for the following purpose:
To develop and implement an anti-doping program among recreational tennis players.
You give your explicit consent with your personal data to be forwarded by the Bulgarian Tennis Federation to third parties where needed for implementing the purpose above.
