Thank you for taking the time to complete ACT’s Service User Satisfaction Survey
This survey is an opportunity for our service users to give us feedback. Please tell us how we are doing, what’s working, and ways we can improve.
This survey is anonymous – you will not be asked to provide any identifying information and you cannot be connected to your answers.
It will take about 10-15 minutes to complete this survey, depending on how much information you want to provide.
The information you share will help us make decisions about ACT and how we provide services. We’ll report back on what we heard from this survey, so stay tuned for the survey results.
