Informed Consent:

Thank you for participating in this survey. Your feedback is important. Please answer the following questions as honestly as possible. The purpose of this study is to help the researcher understand the extent to which conflict in workgroups motivate employees to find work elsewhere.

I do not anticipate that taking this survey will contain any risk or inconvenience to you and it is quite short – just 16 multiple choice questions. Furthermore, your participation is strictly voluntary and you may withdraw your participation at any time without penalty.

All information collected will be used only for my research and will be kept confidential. There will be no connection to you specifically in the results or in future publication of the results.

The first nine questions are taken from Jehn & Mannix (2001). The next four questions are from Klein, Cooper, Molloy, & Swanson (2014). The informed consent text is based on text I found on the University of Wisconsin, Oshkosh website. None of these sources indicated copyright protection.
