This survey assess the heath and human service needs of lesbian, gay, bisexual and transgender (LGBT) individuals living in St. Louis.

For the purpose of this survey, health and human services includes anything that enhances your health or well being. This survey includes questions assessing access to health care and legal services, your mental health, your experiences with violence and victimization, your use of LGBT specific services, and your demographic information. We hope that the information collected from this survey will help in developing new services for the LGBT community.

This survey is anonymous and confidential and you will have the ability to exit the survey at any time. However, due to its nature, the internet will never be completely secure. To insure your maximum privacy, we recommend you complete this survey on a private computer and delete any temporary files or cookies after you have completed the survey.

Please do NOT take this survey if you are under the age of 18 or an undergraduate at Washington University in St. Louis
