1.Introduction
Please take the time to read this before starting the survey.
Limitless Connection Center (LCC), a Marin-based 501(c)(3), is conducting a community survey for people living with disabilities and aging adults. This survey is being administered by Mad Learning Lab, a San Francisco-based community research firm.
This survey was built to capture the experiences of a wide range of people who live with disabilities or the changes that come with aging. That means not all questions may pertain to you, and some may seem irrelevant. Please be patient with the survey tool, as we are trying to capture information from many different people.
We understand that some of these questions may be difficult to answer for someone else. Please do the best you can. We are trying to get baseline information on the experiences of the disabled and aging community in Marin, and your answers help us get there.
Raffle
As a thank you for your time, you can enter a raffle to win one of five $25 Visa gift cards. Entry is optional. To protect your privacy, the raffle is completely separate from the survey. At the end you will get a link to a separate form where you can enter your contact information. Your survey answers are never connected to your raffle entry, so your responses cannot be traced back to you. Winners will be drawn on October 16, 2026.
