Important: This learning collaborative is designed to offer intensive technical assistance and support to participants who wish to achieve Medicare Part B reimbursement for the following benefits: Diabetes Self-Management Training (DSMT), Health and Behavior Assessment and Intervention (HBAI), and Chronic Care Management (CCM). Applicants will be asked to select one benefit for their primary area of focus.

Please read the Medicare Reimbursement Learning Collaborative Charter carefully. It describes the aims, expectations, and support that will be provided to learning collaborative participants.

Number of participants. In collaboration with the Administration for Community Living, the National Council on Aging will select up to ten organizations to participate in this twelve-month learning collaborative. Selection will be based on review of the applications to determine which organizations would most benefit from this type of experience. Organizations should coordinate with others in their designated coverage area or state and identify one organization as the lead on this learning collaborative initiative.
 
Eligible applicants. This learning collaborative is designed for organizations that are in the  early to intermediate stage of developing their business acumen to secure reimbursement for their CDSME programs. Organizations that want to achieve Medicare reimbursement but need additional support to navigate the process should apply. For DSMT, this includes those who want to achieve DSMT accreditation but haven’t yet begun this process; those who recently started their application process; and those midway through the process who need additional support to complete it and to make their programs viable.
 
Applicants and their partners must be committed to the project and willing to provide a considerable amount of time and resources from their organizations to actively participate in this learning collaborative (see “Participant Expectations” in the Medicare Reimbursement Learning Collaborative Charter). Due to the intensive effort that will be required to achieve the aims of this learning collaborative, it is NOT recommended that the Executive Director of an organization be designated as the lead.
 
Organizations that are NOT eligible to participate. Those with accredited diabetes programs or who are already billing Medicare for their CDSME programs; NCOA's 2016-2017 first generation DSMT and HBAI Learning Collaborative completers; 2017-2018 Medicare Reimbursement Learning Collaborative participants; those who are participating in or who have been selected to participate in another business acumen learning collaborative with an overlapping time frame.
 
A kick-off in-person meeting will be held on May 21 from 1:00 pm through 5:00 pm prior to the NCOA Center for Healthy Aging Annual Meeting. For those that are unable to attend the Annual Meeting, webinar capability will be available.
