Report Volunteer Time Form

This form is to be used by the point of contact for your organization to log employee hours. If you utilize an internal system to track hours, please upload a screenshot or PDF of the employee hours logged. Otherwise, please provide documentation of tracking volunteer hours via spreadsheet or other tracking method. Hours can be reported weekly, monthly, or all hours can be reported at the end of the challenge. All hours must be reported by June 8th at 5PM.

Note: If your organization chooses to submit all hours at the end of the Challenge, you will not be included in social media updates highlighting employers with the highest number of volunteer hours throughout the Challenge.

Documentation is required each time you submit volunteer hours. Documentation should include employee names, how many hours they volunteered, the date they volunteered, and where they volunteered.
