Thank you very much for your interest in applying to be considered for a Patient/Caregiver Quality Improvement Partner. 

Quality Improvement is a standardized approach to problem solving. CMH currently provides training to staff and physicians in this standard approach. CMH has recognized the need to have patients at the table co-designing solutions with us.  We are looking to enroll patients and caregivers with unique skills, talents and insights to help co-design various quality improvement processes and initiatives. Selected applicants will receive quality improvement training and will be linked with a mentor to support their ongoing involvement. 

The Patient/Caregiver Quality Improvement Partners are patients and or caregivers who have experienced care at CMH, ideally within the last 2 years. They believe their experience and time can help improve quality at CMH. The estimated time commitment for this volunteer position would be about 24 hours a year or 2 hours per month. 

The information you provide will be reviewed to help in selecting applicants with the skills and experience to support improvement at CMH. 

Requirements for this role are:  

1)  Diverse healthcare experience

2)  Interest in quality and process improvement

3)  Ability to work within a team

4)  Knowledge and experience in quality improvement is an asset

5)  Strong communication skills and ability to share personal experiences

6)  Ability to listen to other points of view

7)  Availability and capacity, ideally looking for a commitment of 1 to 2 years
