My name is Michael Mitchell; I am an officer with the California Highway Patrol. I am a candidate in the Commission on Peace Officer Standards and Training, Master Instructor Certification Course (MICC) class #10. This course is designed to prepare law enforcement instructors to design, develop and deliver a 24 hour course. I am currently purposing a course on PEER support as related to providing assistance to seriously injured officers. PEER support as used in this course is defined as a person offering support due to that person having experienced recovering from a serious traumatic injury and can relate to others who are experiencing similar circumstances.

The purpose of this survey is to determine the necessity of supporting seriously injured employees using a PEER capacity program) from initial injury through home recovery. Your input will assist me in developing a course that would assist those seriously injured employees returning to work through PEER support. . No personal information will be used in this survey. If you have expertise in this subject matter and wish to assist with the development this course, please provide your contact information at the end of this survey. Note: The phrase “serious injury” should be referred to as a “work “ related injury and being hospitalize (more than 72 hours) or off work for more than one month resulting from the following:
• Wounds from gunfire
• Internal injures (heart, lungs, liver, etc.)
• Back injury
• Vision impairment
• Hearing impairment
• Spinal cord injuries
• Amputations
• Severe burns
• Traumatic brain injuries
• Broken or crushed bones
• Respiratory problems
• Cancer
• Joint injuries (i.e., knees, hips, etc.)

Ensure employee's family has transportation to and from the hospital/ rehabilitation center.
Ensure employee's family has resources in place to stay with family/ friends/ hotel while injured employee is in the hospital/ rehabilitation center.
Ensure employee (and family) has resources in place for children to continue attending school functions while they are in the hospital/ rehabilitation center.
Ensure employee (and family) has resources in place to maintain home (home security, maintenance, pets, etc.) while they are in the hospital/ rehabilitation center.
Ensure employee (and family) has resources in place to collect personal mail and make sure household bills are being handled (electric, gas, telephone, etc.) while they are in the hospital/ rehabilitation center.
Ensure out of town family has transportation to and from the hospital.
Provide assistance in understanding workman's compensation forms.

Offer a resource packet that can assist with their transition into medical retirement (similar to resource packet above)
