Mental Health MOTs Questionnaire

Here in the Changing Minds Team at Conwy Mind, we would like to know what you think about our new project, the Mental Health MOTs.
-
Similarly to going to the Dentist for a 6-month check-up, taking your car for its MOT or talking about your physical health, our Mental Health MOTs are an informal measurement of your wellbeing. In the session with our Wellness Check Workers, we can figure out your current wellbeing and together make an action plan going forward. We also aim to identify your strengths and empower everyone who goes to these sessions.
-
We’d love for you to fill out the form below to help us in our mission to end mental health stigma and make mental health checks a normal part of life.

Please take your time answering the questions and if any subject covered in this questionnaire upsets you, please stop the questionnaire and call Conwy Mind on 01492 879907 or contact Samaritans on 116 123, talk to your GP, or if you require immediate support, please contact 999. Any answer given is kept strictly confidential.
1.How would you describe your gender?(Required.)
2.What is your age?(Required.)
Routine check-ups help maintain our cars, teeth, physical health and our pet’s health. Mental Health MOTs can play a similar role in helping us to maintain our emotional wellbeing. Let’s explore your thoughts:
3.Which of these have you done in the last 12 months? (Choose all that apply)(Required.)
4.What encouraged you to go to any of the above appointments? (E.g. due to legal obligations, as someone recommended it, it is the ‘done thing’ or because it was necessary for health etc)
5.If you answered, ‘None of the above’, what was the reason you have not attended? (E.g. cost, time, fear, didn’t think it was necessary etc)
A Mental Health MOT is a proactive check-in for your mind with a mental health worker. It measures your current wellbeing and helps create an action plan to support/maintain your ongoing wellness.
6.Have you attended a Mental Health MOT in the last 12 months?(Required.)
7.If you have answered ‘No’ to the previous question, what has been the reason for not attending a Mental Health MOT?
8.If you answered ‘Yes’ to the previous question, please explain where you were able to receive a Mental Health MOT and what you understand as a Mental Health MOT.
9.Have you recently received any mental health support, or are you on any waiting lists for this support?(Required.)
10.If you were offered a free, informal, Mental Health MOT, would you consider attending an appointment?(Required.)
11.Please explain why you would or would not attend a Mental Health MOT.(Required.)
The co-founders of The London Centre for Applied Psychology (LCAP), identified the need for a routine mental health check-up. They identified five key periods of life in which a Mental Health MOT should be integrated. These are: 18th birthday (leaving home and going to university), moving in with a partner, starting a family, middle age and retirement.

“Just like we take our car to the garage every year to ensure everything’s still roadworthy and in good working order, we should do the same for our mental health."

- Graham Johnston, co-founder of The London Centre for Applied Psychology (LCAP)
