Northwestern Mite Hockey Pre-Registration Form

Please fill out this Pre-Registration Form if your child plans to play Mite Hockey at Northwestern Hockey for the 2012-13 season. Mite Hockey can include a Mite Travel Team or the In House Mite Cross Ice League. This is not a commitment to play in the 2012-13 and no payment is required at this time. This information is being collected to assist with planning for the 2012-13 season.
