This survey will tell us how you feel about our customer services, which services of Spinal Cord Injury Alberta (SCI Alberta) you have received in the last 12 months and the impact these services have had on your life. The results will be presented to SCI Alberta's Board of Directors. They will use the report to make important decisions about the organization's future direction.

All responses are strictly confidential and anonymous. This survey will not affect any services you are receiving from SCI Alberta.

The survey will take about 15 minutes to complete.
