A public-private partnership that connects providers, researchers, leaders, organizations, and advocates, Georgia Center for Oncology Research and Education is led by a Board of Directors of experts from cancer centers, hospitals and educational and academic institutions. Georgia CORE leads statewide advocacy efforts to promote equity in cancer care, funding and policy. We build and nurture collaboration that increases access to innovative resources and research and personalized cancer care and support for patients, survivors, and caregivers.  

Georgia CORE is wanting to expand distribution of survivorship cancer resources around Georgia, especially in underserved areas of the state. With the results of this survey, we are able to better serve cancer survivors within Georgia.

Instructions

•The survey has 22 questions and will take 10 to 15 minutes to complete. If questions are not applicable, please skip and move on to the next question.

•Use the “Prev” and “Next” buttons at the bottom of each page to navigate through the survey. NOTE: “back” and “forward” buttons on your browser will not save responses.

•To leave the survey and save your responses, click “Exit and resume later.” Clicking on the survey link again will bring you back to the last page you were working on.

•When you are finished, click “Done.” At that point, you will no longer be able to enter the survey to edit your responses.
