Background information
The Continuity of Care standard was last updated in 2022. The proposed amendments clarify expectations for follow-up responsibilities and the coordination of patient care to support continuity across Alberta's evolving models of healthcare delivery.
The survey consists of 8 questions, 3 of which are demographic in nature, and should take approximately 5 minutes to complete. It will be easiest to respond to after reading the draft standard (e.g., rationale for edits/inclusions, references/citations, and definitions can be found in the document).

Feedback is compiled, themed and additional edits are considered when finalizing the draft for Council to review and approve for implementation.
